Counseling Application:
Personal Information & Agreement Form

This form gives us basic information about you so that we can contact you more easily in case something comes up that might affect our time together. It also asks you to read, understand, and agree with various aspects of what we do at the Center for Biblical Counseling.

Personal Data Inventory Form

This form asks you to give more personal and background information as well as a short description of why you want to visit with us and what you would like us to help you with. It helps us to get to know you and your situation a little bit before you get here.

3 Things to know or do before the first meeting:

1) The first visit may be a bit longer (depending upon the information gathered), however most meetings will last 50 minutes or less.

2) You could get a huge jump on counseling if you would observe this video and do what Dr. Lawyer suggests before your first meeting.

3) Also, please bring a Bible and something to take notes on to all counseling meetings.
